Incident reports: record an injury and get the parent's signature

Modified on: Thu, Oct 29, 2026, 7:23 AM

When a child is hurt or something happens in the room, record it in Bloomz straight away. The family is asked to read and sign the report in the app, and a serious incident shows when it is due to licensing.

Who this is for

  • Room staff report incidents for the children in their room.
  • Directors and office staff see every report and report serious ones to licensing.
  • Parents read and sign reports about their child.

Staff: report an incident

  1. Open your room, select the star icon, then Incidents in the toolbar. Select + Report incident.
  2. Search for the Child.
  3. Choose Incident or Injury. For an injury, fill in the Body part and Injury type.
  4. Fill in What happened, Where it happened, First aid given and Follow-up.
  5. If it is serious, pick what applies under Serious incident: 911 was called, Poison exposure or Needed medical treatment.
  6. Select Report incident.

Save the report as soon as you can: the time on the report is the time you save it.

Director: follow up and report to licensing

  1. From the center's More menu, under Safety & Wellbeing, open Clinic Visits and the Incidents tab.
  2. Filter with Incident, Injury, Serious incident, Licensing report overdue or Awaiting follow-up. Each row shows whether the parent has acknowledged it.
  3. Open a serious incident. It shows Report to licensing due with the deadline. After you report it, select Mark reported to licensing.
  4. Select Download PDF for a copy to send to licensing or keep on file.

Parent: read and sign

  1. On your home screen, your child's card shows a red To sign chip. Tap it.
  2. On your child's page, tap the Incidents card that says "Needs your signature".
  3. Read the report, sign your name by typing or drawing it, and tap Sign and acknowledge.

Troubleshooting

  • A teacher cannot see a report. Room staff see reports for the children in their own room only.
  • A parent has not signed. The list shows "Waiting for parent to acknowledge". Remind them, or note on the report that you told them in person.
