With the new Forms feature in Bloomz, you can now create, distribute, collect, and manage all types of forms digitally right within the Bloomz platform. Whether it’s a waiver for an aviation program, a health acknowledgment form, or a permission slip for a field trip, Bloomz Forms simplifies every step.
How to Create, Edit, and Manage Forms (District Admin)
Applies to: District Admins
Feature: Forms ( field trip waivers, allergy information, permission slips, program signups etc.,)

Step 1: Enable the Forms Feature
Before creating forms, enable the feature:
Go to District page > Settings
Click on the Features tab
Turn on the Forms toggle
Click Save Settings
Step 2: Create a New Form
You can start creating a form in two ways:
From the Quick Launcher → Create New
From Settings → Forms → Create New Form
Example: Create an Exam Registration Form
Enter the form Title and Description.
Add questions for:
Parent signature
Student information
Mark questions required (use the “Required” toggle).
Choose answer types:
Short Answer
Paragraph
Multiple Choice (Single/Multiple)
Dropdown
Date
File Upload
Add more sections by clicking Add Section.
Choose supported languages (up to 5).
Click Create.
Step 3: Send the Form
You can send forms in two ways:
Option 1: From Form Editor
Choose recipients (district, school, or class).
Set a due date and submission time.
Click Send.
Option 2: From the Forms Section
Go to Settings > Forms.
Select your form.
Click Send.
What Parents See
The form appears in their feed.
Click Fill Form to complete and Submit.
A message will confirm submission.
If allowed, they can resubmit the form.
Step 4: Manage Form Responses
As the form creator, you can:
View submissions: Settings → Forms → Select form → View Results
Edit the form after creation (if not locked)
See individual responses and submission history
Reuse or resend forms for multiple instances
Deleting a Form
To delete a form:
Go to Settings → Forms.
Click on the three-dot menu next to the form.
Select Delete.
⚠️ Note: Only form creators or users with appropriate permissions can delete forms.
