With the new Forms feature in Bloomz, you can now create, distribute, collect, and manage all types of forms digitally right within the Bloomz platform. Whether it’s a waiver for an aviation program, a health acknowledgment form, or a permission slip for a field trip, Bloomz Forms simplifies every step.
A District Admin can create a Form by following these steps -
➕ Create a New Form
You can start creating a form in two ways:
From the Quick Launcher → Create New
From Settings → Forms → Create New Form
Example: Create an Exam Registration Form
Enter the form Title and Description.
Add questions for:
Parent signature
Student information
Mark questions required (use the “Required” toggle).
Choose answer types:
Short Answer
Paragraph
Multiple Choice (Single/Multiple)
Dropdown
Date
File Upload
Add more sections by clicking Add Section.
Choose supported languages (up to 5).
Click Create.

Once created, the form will appear in the Forms section under Settings.
Click the three-dot menu next to the form to Send, Edit, or Delete
