BloomzSend enrollment forms and permission slips to families at your center
Collect enrollment agreements, permission slips and signed consents from families in Bloomz instead of on paper. You build the form once, send it to a room or to the whole center, and see who has filled it in.
Who this is for
- Directors and office staff create and send forms for the whole center.
- Room teachers can send forms to their own room.
- Parents fill in and sign forms on their phone or computer.
Before you start
Forms must be switched on for your center. If you do not see Forms in the More menu, ask your Bloomz contact.
Step 1 — Open Forms
Open your center from Centers in the left rail. Select the … (More) icon in the toolbar. Under Planning, select Forms. You can also type "Forms" in the Search features box.
Step 2 — See what you have sent
The Forms screen has two tabs. Sent Forms lists every form you have sent, with the chips All Forms, Open, Submitted and Closed to filter it. Form Templates holds forms you can reuse.
Step 3 — Create a form
- Select + Forms. The Create Form screen opens on step 1 Define.
- Type a Title, for example "Field trip permission".
- Switch on Requires Signature? if the parent must sign the form.
- Switch on Requires Student Information? if each answer must be tied to a child.
- Select Next and add your questions on step 2 Add Questions.
Step 4 — Send it and follow up
Send the form to families. It then appears under Sent Forms, where Open shows forms still waiting for answers and Submitted shows answered ones.
Enrollment packets
To bundle several forms into one enrollment packet for new families, open Enrollment Admin (More, under Administration). Its chips Packets, Completion and SIS track who has finished, and + New Packet starts a new one.
What parents see
- On a phone, the parent taps … in the bottom bar, then Forms.
- The form you sent is listed under Sent Forms. The parent opens it, answers, signs if asked, and submits.
Troubleshooting
- The Form Templates tab says "No data available". Your center has no saved templates yet. Build the form with + Forms.
- A parent cannot find the form. Check that it was sent to the room their child is in, and that the parent has joined Bloomz. Invited parents who have not signed in yet cannot answer forms.
